Our Care Navigators can assist you with your request to register with the Practice. We hope you will soon get to know how we work! It can take up to 2-3 weeks to be fully registered on the system so we always advise that you have enough medication to see you through this registration period. These can be obtained from your previous practice. 

New patient health checks are sometimes advised for patients with multiple diagnoses and medications so we can ensure that care is continued appropriately. You will be notified if this applies to you.

You can pick up an application form from the surgery reception take it away to fill in and then bring back to the surgery, or download the form from the link below and bring that to the surgery filled in. which you can download below or collect from reception upon registration request. 

When you come to the practice to register with your completed forms, we ask you to bring 2 forms of ID, one photo identification and one with proof of address on if possible.

We ask that each adult and any children over the age of 12 are present when registering at the practice. Children under 12 years old can be registered by the parents without them being present.  We acknowledge and accept that this may not always be possible but we are here to help and your registration will still be welcome.
